Wedding Package
In order to complete your wedding package, we will need the following information from both parties: birth date (month, day, year), time of birth, and city and state of birth, or city and country of birth. We will also need the past life crossing form cmopleted and sent in.

To complete this in-depth analysis, we will need the following information: Your birth date (month, day, year),time of birth, city and state of birth, full name as it appears on your birth certificate and your married last name if different, a copy of your palm prints and finger prints, and your birth sequence. We will also need the past life form filled out and sent in.
In order to process your Career Kit #1, you will need to mail in photocopies of your palms. Please photocopy both the right and left palm, and finger prints. Several copies of each palm with different lighting shades will be helpful. For an accurate viewing of the fingerprints, please do a separate photocopy with only the finger prints. In addition, you will need to provide the following information on the next order form page:
Complete name on birth certificate
Married name and or nick names
Date of birth
Time of birth
City, state and country of birth
Your birth order sequence
Career Kit #2 does not require your birth sequence information or palm prints.
